Health & Legal Forms
- Health Form must be signed and dated by the physician within 90 days of travel to be valid. For example, if a program begins on July 1st, you cannot use forms that were completed/signed prior to March 1st.
LEGAL FORMS
- Adult Legal Form (AL)
- Adult Legal Form – DOMESTIC (Local/Regional/National Programs Only)
- Child Traveling Alone Legal Form (CTAL)
- Child Traveling with Leader Legal Form (CTLL)
- Youth Legal Form – DOMESTIC (Local/Regional/Naional Programs Only)
For CISVers, there’s no avoiding the confusing array of forms. And among the most important of those forms are the Health Form and Legal Forms.
But how do you decide which is the correct form, given all the different categories of programs and participants and all the different types of forms?
There are always three variables to consider.
There are two categories of programs:
- International Programs (Village, Step Up, Interchange, Seminar Camp, Youth Meeting, International People’s Project)
- Local/Regional/National Programs (e.g. Selection Overnights, Regional Mini-Camps, CISV In Motion, etc.)
There are three categories of participants:
- Village, Step Up, Interchange and 12-15 year old Youth Meeting delegates, who travel with a leader
- JCs, 17-year-old Seminar Campers, and 16 – 17 year-old Youth Meeting delegates, who travel independently
- Everyone Else: 18-year-old Seminar Campers; Youth Meeting delegates age 18 and up; IPP participants; leaders; Interchange junior leaders; program staff; program junior staff)
There are six types of forms:
- Health Form
- Adult Legal Form (AL)
- Child Travelling with Leader Legal Form (CTLL)
- Child Travelling Alone Legal Form (CTAL)
- Adult Legal Form – Domestic
- Youth Legal Form – Domestic
So which form is the correct one?
For the Health Form, the answer is simple:
- The Health Form is required for every participant at an International program.
- The Health Form is not required for any participant at a Local/Regional/National program.
The correct choice among the five Legal Forms depends on two things:
- Whether the program is International or Local/Regional/National.
- The age of the participant. The role of the participant is irrelevant; it’s the age that matters.
- Delegation members age 11 through 15 at International programs use the Child Traveling with Leader Legal Form (CTLL). This means Village, Step Up, Interchange, and 12-15 year-old Youth Meeting delegates.
- Participants age 16 and 17 at International programs use the Child Traveling Alone Legal Form(CTAL). This means JCs, 17-year-old Seminar Campers, and 16-17 year-old Youth Meeting delegates.
- Participants age 18 and older at International programs use the Adult Legal Form (AL). This means 18-year-old Seminar Campers, Youth Meeting delegates age 18+, IPP participants, leaders, Interchange junior leaders, staff, and junior staff.
- Participants age 17 and younger at Local/Regional/National programs use the Youth Legal Form – Domestic.
- Participants age 18 and older at Local/Regional/National programs use the Adult Legal Form – Domestic.
- NOTE: USA participants (delegates, leaders, JCs, and staff) at USA-hosted International programs (Village, Step Up, Interchange, Seminar Camp, Youth Meeting, International People’s Project) always use the International forms (Health, CTLL, CTAL, AL), not the USA forms (Adult Legal Form – Domestic and Youth Legal Form – Domestic).
