Chapter Volunteers · Chapter Membership (Individual Members $30 / Family Members $50) · Consent for Background Check | pdf (needs notary signature / must be redone every 3 years) · Driving History Report (MVR) (for last 3 years / must be age 25 or older to drive CISV participants) · Proof of Auto Insurance - anyone driving others for purposes of a CISV program must provide their Chapter’s Insurance Chair with an annual Motor Vehicle Report and a cop of his/her personal auto policy with liability coverage of at least $100,000.
